1. Book a Test using Website Book an Appointment
- You can book a test from our website https://lifenitywellness.com by clicking on “Book an Appointment” button.
- As per your convenience you can select either “Home visit” or you can “walk in” to your nearest Lifenity Collection Centre.
- Provision is done to enter your Name, Mobile Number, Location
- If you know the test to be done , you can select it from drop down option from “Select Test”
- We have provided option “Upload Prescription” on website to upload prescription provided by your doctor.
- Click on “ I agree to Lifenity T&C and Privacy Policy
- Click on Submit Button.
- By clicking on submit button you have submitted your Test request.
- Our Customer care team will call you and appointment will be confirmed and all relevant details shall be provided on call.
- All details related to Sample collection will be confirmed by SMS / WhatsApp message.
- For Walk In centre and Time Slot will be informed on call as well as by SMS / WhatsApp message.
2. Book a Test by Calling Customer Care
- You can call our customer Care number +91 8657379999 (Available 365 days and 24 X 7).
- On call you can share your required details like Name, Mobile Number, Location, Test to be conducted etc
- Our Customer care team will share you details of confirmation of appointment on call.
- For Home Visit: Sample collection time and location will be confirmed on call as well as by SMS / WhatsApp message.
- For Walk In: Collection centre Address and Time Slot will be informed on call as well as by SMS / WhatsApp message.
3. Book a Test by Sending WhatsApp (You Can Click on Website Whatsapp Number to connect to Us)
- You can send your details like Name, Mobile Number, Location, Home Visit/ Walk in, Test to be conducted , Doctors Prescription on Whatsapp
- Our Customer care team will share you details of confirmation of appointment on call. And reverting on Whatsapp
- For Home Visit: Sample collection time and location will be confirmed on call as well as by SMS / WhatsApp message.
- For Walk In: Collection centre Address and Time Slot will be informed on call as well as by SMS / WhatsApp message.
Sample Collection
As per the Schedule time our Expert Phlebotomist will reach to your home, informing you in advance. The Phlebotomist are well trained to collect samples. Phlebotomist will follow all safety rules and guideline.
Sample Testing
The collected samples will reach to the Lab for further processing. The well Trained Lab Technicians perform test and Report will be sent for validation.
Reporting
The Test Report shall be shared on the registered email id and link is sent on SMS on your Registered Mobile number to download report. The Turnaround Time is met as committed while booking a test. (For example : The RT-PCR report is delivered within 12 hours and Rapid PCR test Reports are delivered within 2 Hours.)
